For patients with chronic diseases, such as congestive heart failure or chronic obstructive pulmonary disease, Community Nursing Services of North East may provide patients with the McKesson Telehealth Advisor™, a personal, in-home daily information link with the agency. The system sends important healthcare information right to your home and, in turn, transmits information about your condition to the agency every day. Made up of an in-home Health Buddy® appliance; interactive health management information and an advanced monitoring application, McKesson Telehealth Advisor gives you the individualized healthcare education you need to help you gain control over your symptoms and enjoy a healthier life.

McKesson Telehealth Advisor provides detailed health management programs to help you learn
how to take better care of yourself. Each program addresses a specific diagnosis or combination of diagnoses. Every telehealth session is tailored to your specific needs based on your previous responses. For instance, if your answers indicate some confusion about a specific symptom, the system sends education designed to help improve your understanding and what you can do to reduce the severity of the symptom.

Taking the form of questions and interesting trivia that’s relevant to your diagnosis, the system
makes it easy to learn and improves retention of important information. The information and
reinforcement the system puts you in control of your own well-being; empowering you by
teaching you how to avoid symptom exacerbations and possibly may decease visits to the doctor’s office, emergency room or even a hospital stay.

The Health Buddy appliance is a convenient and easy-to-use in home data collection appliance. It plugs into a basic electrical outlet and uses your existing telephone line to receive and transmit education and questions to you every day. You answer the questions
using four large buttons. As part of your telehealth session, vital sign information such as blood pressure, weight and oxygen saturation readings are electronically transmitted by a connection to the Health Buddy device.

An average telehealth session lasts less than eight minutes. Once it is completed, the Health
Buddy appliance sends the information to Community Nursing Services of North East over your telephone line. It does not interfere with your telephone service and there is no extra charge for the call.

responses and vital sign data daily, the system gives Community Nursing the up-to-date information about your condition in order for the agency to help you continue to enjoy the best possible health. This eliminates the gaps in communication that can often negatively impact your well-being. People find this daily interaction very comforting and say it gives them a sense of security to know that someone is concerned about their well-being.

The information you send during your telehealth session is analyzed by qualified agency personnel. If you begin to show signs of possible complications, staff can respond immediately by calling you or notifying your physician. All of the information you send during the telehealth session is completely secure and confidential.
