Community Nursing Services of North East hospice team relies on the combined knowledge and skills of an interdisciplinary team composed of the following members:

The patient's personal physician
Hospice medical director
Nurses
Hospice aides
Social workers
Clergy and other counselors
Trained volunteers
Bereavement Counselor
Speech, physical, and occupational therapist as appropriate

The team coordinates an individualized plan of care for each patient and family. These services are designed to bring comfort, peace and a sense of dignity at a very trying time. Hospice reaffirms the right of every patient and family to participate fully in the final stages of life.
